- A valid, unexpired photo ID — driver’s license, state ID, passport or military ID.
- Any fingerprint cards your agency mailed you — if they sent specific cards, bring them; otherwise we can provide standard FD-258 cards.
- Your reference or ORI information — any code, form number or agency address the requester gave you.
- The correct number of cards — let us know how many completed sets you need.
